Position Overview

We are seeking a Board-Certified Family Medicine Physician to join a growing outpatient practice focused on delivering high-quality, patient-centered care. This role emphasizes preventive care, chronic disease management, and collaboration within a multidisciplinary team.

Key Responsibilities

• Provide comprehensive primary care to patients of all ages

• Manage a patient panel in an outpatient setting (20–25 patients/day)

• Diagnose and treat acute and chronic conditions

• Develop and implement care plans focused on preventive health

• Utilize EMR systems (preferably Epic) for documentation and care coordination

• Collaborate with nurses, specialists, and care coordinators

• Participate in quality improvement and value-based care initiatives

Required Qualifications

• MD or DO degree from an accredited institution

• Board Certification in Family Medicine (or eligibility)

• Active Indiana medical license (or ability to obtain)

• DEA certification

• Strong communication and patient engagement skills

Preferred Qualifications

• 3+ years of experience in a primary care or outpatient setting

• Experience with Epic Systems

• Background in value-based care or population health

• Demonstrated success in improving patient outcomes
